Site History
The Horizon Healthcare for the Homeless Program was established in October 1987 through a management contract with the County Health Department. The name, Horizon Health Center, was adopted in December 2000 when Advance Community Health (formerly Wake Health Services) assumed full responsibility for the medical practice.
