What is complex care?
Advance Careplex at Beacon Point improves health and well-being for people with complex health and social needs by coordinating and altering care delivery at the individual and community levels! Our center addresses root causes of unmanageable health through interdisciplinary care teams and cross-sector partnerships that deliver patient-centered care based around participants’ own goals and priorities.

- Ability to see providers at Advance Community Health Adult Clinic
WHO IS QUALIFIED TO RECEIVE TREATMENT?
Patients with complex medical issues such as, but not limited to:
- Frequent hospital visits
- Uncontrolled medical conditions (DM, HTN, COPD)
- Cancer diagnoses
- Congestive heart failure (CHF)
- Patients with adverse social determinants of health issues, e.g., homelessness

In 2024, we proudly joined forces with the UNC Physicians Network to lead and manage complex care services at Advance Careplex at Beacon Point. This collaborative and mission-driven partnership has empowered us to enhance health and well-being for individuals facing complex health and social challenges. Our complex care center transforms care delivery on both individual and community levels, addressing underlying causes of health issues through interdisciplinary care teams and cross-sector partnerships. We are dedicated to delivering patient-centered care that revolves around participants’ own objectives and priorities. Our approach recognizes that these root causes often transcend physical health, encompassing social determinants as well.
