Are you ready to take control of your health? Join DiaBeat It!, our Diabetes Management Program. Tailored to meet the unique needs of our community, this culturally relevant program offers a holistic approach to managing diabetes and aims to give patients the tools and support needed to make lasting lifestyle changes.
Program Details
Led by a certified wellness practitioner, this 3-month program guides patients through a variety of health education sessions and activities including:
- Personalized Diabetes Education
- Regular A1C Monitoring
- Nutrition Education
- Mental Health Education
- Grocery Store Tours
- Cooking Demonstrations
- Medication Management
- Group Fitness Sessions, and more!
Upcoming Cohorts
The next cohort will begin in January 2026.
Class meets in person on Monday’s from 6PM – 7PM at the Southeast Raleigh YMCA (1436 Rock Quarry Rd, Raleigh, NC 27610).
Class will be held weekly for the first half of the program and bi-weekly for the last half.
